| Using your TruAssure vision care plan is easy! | |
| 1. | Have your TruAssure Information Card available when scheduling an appointment or visiting a TruAssure In-Network Provider. It’s very important that your In-Network provider know that your TruAssure vision care plan is administered by EyeMed Vision Care. |
| 2. | Present your TruAssure Information Card at the time you receive service or materials from a TruAssure In-Network Provider. Pay your copayment and any other charges not covered by the vision care plan at the time of service. No paperwork is required. You continue to save on additional eyewear purchases any time you present your card to a TruAssure In-Network Provider. |

If you select a Provider who is not in TruAssure’s network, you do not receive TruAssure
preferred pricing and you should provide full payment to your Out-of-Network Provider
at the time of service. To receive benefit reimbursement, submit a completed
Vision Claim Form, along with itemized receipts from your Provider and your
prescription to:

Pre-authorization is not required but if you want to be pre-authorized for vision
care benefits, please contact TruAssure toll-free at 1-866-723-0513. The Customer
Care Center is available Monday through Saturday 7:00 a.m. to 10:00 p.m. CST and
Sunday from 10:00 a.m. to 7:00 p.m. CST. After hours, please leave a voice mail
request, including patient name, Employee Social Security Number or ID, the requested
services and your daytime telephone number. We will confirm your eligibility for
